I have reviewed the MTDA information web-page with my child and support my child’s participation and enrollment in MTDA online courses.

I have reviewed the Butte School District Policy 2170 relating to Distance Learning and online courses.

I understand that my child will need access to a computer with Internet in order to participate in online MTDA courses.

I understand there is computer availability at high schools during the following hours:

8:00 a.m.-3:30p.m. Monday –Thursday
8:00 a.m.- 3:00 p.m. Friday

Students must submit this form to their school counselor prior to registering for MTDA courses. Provide the name of the course(s):
